Peri-implantitis – adjunctive measures to non-surgical peri-implant therapy

This review of the effectiveness of patient-performed or administered adjunctive measures to non-surgical peri-implantitis therapy included 9 RCTs. A wide range of interventions were tested but the studies were mainly small with only one being at low risk of bias. Consequently addition high quality well conducted studies are needed on both the prevention and treatment of peri-implant disease.

This review comparing the effectiveness of different surgical methods for peri-implantitis included 13 parallel group RCTs. However, a wide variety of interventions were tested and the studies were generally small providing very low certainty evidence. Consequently additional high quality well reported studies are needed.

Bioactive glass for periodontal defects

This review aimed to assess the effect of bioactive glass on bone and periodontal regeneration and included 20 RCTs. The included studies were small, involved numerous comparisions with only two being at low risk of bias. The findings showed a statistically signiciant benefit for probing depth with bioglass but this may not be clinically significant and the limited quality of thei included studies mean the findings should be interpreted cautiously.

Adjunctive chlorhexidine for gingival health in orthodontic patients

This review of the efficacy of chlorhexidine (CHX) products in maintaining gingival health in orthodontic patients with fixed appliances included 20 RCTs only 1 of which was at low risk of bias. The findings showed a benefcial effect on gingival parameters with CHX mouthwash at 1-3 months but the findings should be interpreted cautiously.

Peri-implant mucositis – patient performed adjunctive measures

This review of patient-performed or administered measures used adjunctively to submarginal instrumentation for the management of peri-implant mucositis included 14 studies. The findings indicated that adjunctive measures reduced probing depth, bleeding on probing and plaque level reductions at 3 months and were in favour of adjunctive measures, which were plaque.

Air polishing in supportive periodontal treatment

This review comparing of air polishing devices with conventional periodontal therapy (hand instrumentation and/or ultrasonic debridement), in patients undergoing periodontal maintenance therapy included 14 RCTs. The findings suggest that they have comparable clinical efficay although the evidence is of low certainty.

Peri-implantitis: Are locally administered antibiotics effective?

This review of the effectiveness of locally administered antibiotics in the treatment of peri-implantitis included 12 small studies 7 of which were RCTs. While the findings suggest a small additional benefit with antibiotic use however the clinical importance of this is debatable.

This review comparing the effects of vacuum-formed retainers (VFR) and Hawley retainers (HR) on periodontal health included 6 RCTs. While the findings suggests a statistical benefit in favour of HR in the short term the studies are small, short-term and most ( 5) were considered to be at high risk of bias.

Diabetes and dental implants

This review of the possible association between diabetes mellitus and dental implant complications included 10 cohort studies (8 prospective 2 retrospective). The findings suggest that patients with diabetes are more likely to have clinical complications with dental implants.
